Face line
A V-line without bone surgery: when fat removal is the better answer
Many people who look into facial contouring surgery are actually bothered by fat or sagging rather than bone. Target Plastic Surgery does not cut bone — it slims the face line with fat removal, threads and grafting — so the first question at consultation is always: what is making your face look wide?
| Approach | Fat removal + thread lifting (+ fat grafting where volume is lacking) |
|---|---|
| Bone cutting | None |
| Anaesthesia | Sedation plus local anaesthesia |
| Typical return to daily life | From the next day (face washing, make-up) |

Four reasons a face looks wide
Bone
Wide cheekbones, a square jaw angle or a long chin. Only bone surgery changes the bone itself.
Muscle
A large chewing (masseter) muscle at the jaw angle. Botox is often used to reduce it.
Fat
Fat under the chin, in the lower cheeks (jowls), in front of the cheekbone (malar fat) or over the side cheek.
Sagging
Skin and fat that have dropped with age blur the jaw line. Lifting is the tool here.
Most faces are a mix. The surgeon examines bone, muscle, fat thickness and skin firmness together before recommending anything.
What “contouring 3-point” surgery does
In Korea, facial bone contouring usually means zygoma (cheekbone) reduction, mandible angle reduction and chin surgery, sometimes done together. These operations change the bone framework, are done under general anaesthesia and involve a longer recovery. They are the right choice when bone is the main cause of width — but they do not remove fat, and front-of-cheek volume beside the nose is often unchanged after cheekbone reduction.
What fat removal and threads can do
- Double Layer facial liposuction reduces fat in the double chin, jowls, malar area and side cheeks — the parts of the face line made of fat.
- A thread lift lifts sagging cheeks and jowls and supports the skin over the new contour.
- Jaw Botox can be added under the same sedation when the chewing muscle is large.
- Fat grafting restores volume where the face is hollow, so a slimmer face does not look tired.
Experience with the whole facial structure
Dr. Seo’s earlier surgical work included facial contouring, facelift, thread lifting and fat grafting. That experience shapes how he plans fat removal: the line is designed around the bone framework, so the result follows the natural structure of your face.
If bone is the main cause of the width you want to change, fat removal will not give the result you are looking for — and you will be told so at consultation.
Frequently asked questions
Does Target perform zygoma or jaw reduction?
No. The clinic focuses on fat removal, thread lifting and fat grafting. Bone contouring is outside its scope.
How do I know whether my problem is fat or bone?
The surgeon examines you in person, pinching and assessing fat thickness, muscle size and bone shape. Clear photos sent before your visit help with an initial opinion, but the decision is made in person.
Can I have liposuction after contouring surgery?
Fat removal is sometimes considered after bone surgery to refine the line. Bring your surgical history to consultation so the surgeon can assess it.
